| || JOB OVERVIEW
Under the direction from the supervising social worker, the Clinical Social Worker II (CSW II) at the Ward 86 HIV/AIDS Clinic at Zuckerberg San Francisco General Hospital (ZSFG) will work with individuals and families on an on-going basis to provide comprehensive social work services including but not limited to psychosocial assessments, assisting patients in finding housing, substance abuse counseling, mental health assessments, and appropriate referrals to community agencies and legal/immigration resources. The CSW II will provide a full array of social services that include, but is not limited to, crisis interventions, provider consultation, insurance and financial counseling and facilitate multi-disciplinary case conferences monthly in which care plans are developed for high-risk patients. The CSW II will be required to participate in social work meetings, staff meetings, case conferences and other required Ward 86 clinic meetings.
DIVISION OF HIV, INFECTIOUS DISEASES AND GLOBAL MEDICINE
Ward 86 (formerly known as The Positive Health Program), the HIV/AIDS Clinic at the Zuckerberg San Francisco General Hospital (ZSFG) campus has been a leader and innovator in field of HIV care since the beginning of the epidemic. We are the largest clinic providing primary medical care for people living with HIV for the San Francisco Department of Public Health (SFDPH) and also provide preventive services to patients at-risk for HIV infection, including pre-exposure prophylaxis (PrEP). We provide care to ~2500 patients living with HIV, with over 28,000 preclinical encounters a year. Services are offered through a comprehensive, multi-disciplinary approach, addressing primary medical and HIV care, urgent care, mental health evaluation and treatment, substance use assessment, treatment and referrals, HIV prevention services, HIV specialty care needs (including on-site Pulmonary, Dermatology and Cardiology clinics), and psycho-social assessment and interventions.

| || • Master's degree from an accredited school of social work, and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position
• Bilingual in Spanish - Ability to speak and communicate effectively with Spanish speaking patients
• Experience in working with people living with HIV/AIDS
• Experience in working with marginalized patient populations and culturally diverse populations including the LGBT community
• Experience in working with patients with mental health and substance use problems
